We're using Migradoc to generate a Fax cover sheet containing a few items of information.

My client would like the cover sheet template to be user defined, that is, they supply us with a PDF containing placeholder for each item of data. What is the best approach for doing this as we essentially need to scan their PDF document for the relevant placeholders - I figured Acroform Fields may be the best option?

Acroforms are not my area of expertise. I would use "normal" PDF files plus a separate text file that lists positions, dimensions, and variable names of the data that have to be filled in.
