I have some PDFs that act as templates and I need to loop through all of the images in the PDF and replace them. So the first question is how do I get a reference to an image in the PDF and secondly, is there any way I can label the images with bookmarks or something to make it easier to figure out which image I am dealing with?

The EXPORT IMAGES example shows how to get a reference on the images which are /XObject with /Subtype = /Image, no idea about the second part of your question, sorry.

Maybe try to replace the Stream of that Element.
